At Vital Telecare, we believe patients should have access to quality healthcare, no matter what their address is. Illnesses can arise anytime, and providers should serve as an expert to help manage these illnesses at the patient’s convenience. We want to make healthcare easy, convenient, and affordable.

Our Story

Vital Telecare was first conceptualized in 2019 when our founder, Rashmi Momin, sought to provide cost-effective and convenient care, without compromising on quality. In response to patient requests and dissatisfaction with long wait times for in-office visits, Rashmi researched various options to meet her patient needs. During her Doctor of Nursing Practice (DNP) studies at the University of Texas Health Science Center, Rashmi found telemedicine to be a promising model for the future of healthcare delivery. As our company was taking shape, COVID-19 accelerated its launch, as the demand for telemedicine skyrocketed. We haven’t looked back since and are excited to offer expert telemedicine services to everyone in our virtual community!

Meet the Providers

Rashmi Momin

Rashmi Momin is a Board Certified Family Nurse Practitioner, who specializes in Internal Medicine and Family Practice. She attended Baylor University in Waco, Texas where she received a Bachelor’s in Business Administration. In 2008, Rashmi pursued her interest in nursing and graduated with honors, from the Accelerated Bachelor of Science in Nursing program from the University of Texas Medical Branch. While working in various intensive care units, she completed her Master of Science in Nursing with a subspecialty in Family Practice. Rashmi is currently pursuing her Doctorate of Nursing Practice (DNP) degree at the University of Texas Health Science Center. She has worked primarily in Internal Medicine settings and is involved in the entire continuum of care of patients.
Rashmi is an active member of several professional organizations including the American Association of Nurse Practitioners; Sigma Theta Tau, the International Honor Society of Nursing, and the Houston Area of Nurse Practitioners.
Rashmi and her husband, Tanveer, live with their two children and a fur baby in Sugar Land, TX. She enjoys DIY projects, reading, cooking, and traveling around the world.

Mubarak Khawaja, MD

Dr. Khawaja is a board-certified internal medicine physician with training in pulmonary medicine and has over 30 years of experience in acute and chronic health care management. He currently practices at CareOne Internal Medicine and Primary Care and aims to provide compassionate, high-quality care for all patients in and around Houston and Katy, Texas.
Dr. Khawaja received his medical degree from Liaquat University of Medical & Health Sciences in Sindh, Pakistan, in 1987. In 1994, he completed his residency in internal medicine at Mercy Catholic Medical Center in Philadelphia, Pennsylvania, through the Medical College of Pennsylvania, now known as Drexel University College of Medicine, where he also served as chief resident in 1995. He received further training through his fellowship in pulmonary medicine at Allegheny General Hospital in Pittsburgh, Pennsylvania, also through Allegheny University of Health Sciences (now part of Drexel University College of Medicine) in 1997.
After his training, Dr. Khawaja served as a Clinical Assistant Professor for the Department of Family Medicine at the University of New Mexico for seven years. He then relocated to Katy, Texas, in 2006. Dr. Khawaja is affiliated with Memorial Hermann Katy Hospital and Methodist West Houston Hospital. He is a member of the Texas Medical Association, the Harris County Medical Society, and the American College of Physicians. To better serve more patients in his community, Dr. Khawaja is fluent in Spanish, Hindi, Urdu, and Gujarati.
Dr. Khawaja currently lives in Sugar Land, Texas, with his wife Sakina, and their two children. In his spare time, he enjoys philanthropy and traveling.
